Make a written submission

Adoptees or adoptive parents can fill out this form to tell the investigation about their experiences.

About the written submissions form

This form is for adoptees or adoptive parents who want their lived experiences considered in the final report of the Independent Investigation into Historical Intercountry Adoptions from the Republic of Korea to Australia.

Your adoption must have taken place from the Republic of Korea to Australia between 1964 and 1999.

The investigation will not make findings on individual cases. It will look for patterns across all cases it considers.

Submissions close on 4 December 2026.

If you have any questions about the form, please email ICAInvestigation@dss.gov.au.

Organisations and third parties* can also share information, but the form is only for adoptees and adoptive parents. Please email your contribution to ICAInvestigation@dss.gov.au.

*Third parties may include individuals with the experience of being social workers, departmental staff, policy makers, researchers, or adoptee’s friends.

Filling out the form

The form will ask you questions about:

  • whether your adoption is within the scope of the investigation
  • the circumstances of your adoption
  • your lived experiences, particularly what you know about the processes and procedures related to your adoption.

What you can tell us

Each Australian state and territory has different laws relating to its adoption files.

The form will ask you different questions if your adoption took place in New South Wales. The state has strict laws against sharing details from your adoption file.

If your adoption took place in another state or territory, you may choose to share information from your adoption file.

You do not need access to your file

You can submit your written statement even if you don’t have access to your adoption file. We still want to know about your experiences.

You can still make a written submission if you have already submitted an Expression of Interest to have your file reviewed.
