Review of HACC Service Group Two

The Department is currently reviewing activity under the HACC Service Group Two which includes the service types:

  • Assessment
  • Client care coordination
  • Case management, and
  • Counselling/support, information and advocacy for both the carer and the care recipient.

This is being undertaken to inform the design of the Commonwealth Home Support program so that decisions regarding future arrangements for the HACC Service Group Two service types are based on comprehensive and accurate information about how services are currently delivered at the local level.

Survey

All HACC providers delivering services to older people were invited to participate in an online survey to capture information on assessment, client care coordination, case management and counselling/support, information and advocacy (care recipient and the carer) within the Commonwealth HACC Program.

The information collected through the survey is helping inform development of the Commonwealth Home Support Program to be introduced from 1 July 2015.

The survey closed on Friday, 28 June 2013.

Consultations

We are keen to hear in person from service providers funded to deliver services under HACC Service Group Two. The focus of the consultations will be on capturing the diversity of activities that service providers currently deliver under these service types. Information regarding future funding arrangements for these activities will be made available by early 2014.
